Shelter Case Manager $23-$25 per hour Shift: Wed - Sun 2:30pm -
11pm SUMMARY: Under the supervision of the Shelter Administrator,
the Case Manager plays a crucial role in supporting individuals
experiencing homelessness. The Case Manager assists individuals in
developing their individualized housing plan and provides tailored
resources to support their stability and self-sufficiency To
support these efforts, the selected candidate must establish and
connect with community resources that offer diverse services to
fulfill the participant's goals and schedules. The Shelter Case
Manager will be responsible for the Duties and Responsibilities
outlined below and other duties as assigned. DUTIES and
RESPONSIBILITIES: ? Complete participant intake process including:
screenings, assessments, plans, etc. ? Develop a network of
community-based resources to refer participants to, including
housing options, substance use treatment, medical services,
education, and employment ? Build relationships with property
managers and landlords to maintain a local housing inventory. ?
Assist participants with applications, research, and connections to
help them achieve their goals (e.g., health insurance, benefits
enrollment, housing subsidies (HUD’s Housing Choice Voucher Program
(Section 8)) ? Must uphold core values of integrity, transparency,
accountability, respect, and responsibility. ? Work closely with
other staff, volunteers, and partners to ensure consistent care and
support to participants ? Must maintain and handle participant
information professionally and with confidentiality ? Advocate for
participants with landlords, service providers, and community
organizations to help them achieve their goals ? Establish
procedures and contacts for housing retention, including crisis
plans and early intervention services. ? Assist participants with
moving logistics and identify ways to cover the expenses of
security deposits and moving costs. ? Assist participants with
securing vital documents (e.g., Social Security cards, birth
certificates). ? Provide services following best practices for
individuals experiencing homelessness with complex health,
disability, or behavioral health conditions. ? Provide accurate and
timely documentation of participant progress following any required
standard in HMIS and other data collection platforms. ? Any other
tasks assigned by the Shelter Administrator. QUALIFICATIONS: ?
Deliver compassionate, respectful, and culturally sensitive care to
all participants. ? Foster a welcoming and supportive environment
through a professional and friendly demeanor. ? Must be able to
communicate effectively and accommodate diverse cultural and
linguistic needs to ensure participant understanding ? Must be
familiar with local resources and housing systems ? Address
participant concerns with empathy and attentiveness. ? Respect
cultural differences during interactions with participants. ?
Maintain a positive attitude, demonstrating patience and resilience
in stressful situations. ? Practice active listening to ensure
participants feel heard and their needs are addressed. ? Bilingual
(Spanish/English) preferred. ? Proficiency using Microsoft
Office/Google Suite products and client database systems (HMIS) ?
Must complete a background check, physical, TB test, and drug
screen before hire. EDUCATION/EXPERIENCE: ? High School Diploma or
equivalent qualification required. ? Experience working with
individuals with diverse needs, including housing status,
disabilities, behavioral health, and medical conditions ? 2-3 years
relevant experience in Housing Navigation and/or related Case
Management services preferred. ? Valid, unrestricted CA driver’s
license. ? Certification in Basic Life Support (BLS) from the
American Heart Association (AHA) or the American Red Cross. For
