the agency’s Continuous Care program by effectively monitoring and
implements strategic plans to help develop the department under the
direction of the Director of Clinical Services & Chief Clinical
Officer in order to better service our patients and families. The
Elizabeth Hospice is San Diego's oldest non-profit hospice of 45
years. We are growing and looking for talented individuals to join

San Diego County and south Riverside County with multiple hospice
office locations. Our Mission: To enhance the quality of life for
those nearing the end of life's journey and for those who grieve.
Salary: $75,000 - $90,000 annually, subject to experience and
qualifications. Work Schedule: Full-Time: (5) 8-hour shifts or (4)
10-hour shifts, days and hours may vary including, two weekend
shifts per month. Work Location: Escondido, Ca. Travel: Travel
required for supervisory patient visits. Occasional travel for
educational purposes. Requirements: JOB DUTIES/KNOWLEDGE/JOB
PERFORMANCE: Implements strategic plans and protocols for the
agency’s Continuous Care program in order to assure compliance with
all federal, state, and local health care regulations. Oversees
budget variances and is accountable for results by reporting out on
lost revenue and monitoring Continuous Care staff allocation to
home care teams in which they provide service. Assists hiring
managers with hiring process by interviewing and assessing
candidates and providing recommendations on hiring decisions.
Provides indirect supervision in collaboration with CCO, DCS or
Manager on call in order to properly assess and evaluate the
clinical practice and performance of the Continuous Care team.
Directly supervises and evaluates the clinical practice and
performance of the Continuous Care team, including training and
performing Field Observation Visits. Participates and offers
recommendations in plan of care decisions and keeps informed of
treatments and management plans of patients on Continuous Care. In
collaboration with the Triage Manager or DCS, administratively
oversees and supervises the planning, coordination, and delivery of
the agency’s Hospice Continuous Care plan. Collaborates with
Compliance and Quality Assurance Departments to investigate and
resolve quality concerns. Collaborates with interdepartmental
managers in order to identify areas of process and performance
improvement needed on the Crisis Care team. Manages and submits
Continuous Care billing to the finance department on a monthly
basis. Collaborates and supports the Hospice Chief Medical Officer
in research projects. Supports the Sapphire after-hours team by
working collaboratively with all Clinical Managers including the
Sapphire Clinical Triage Manager. Participates in agency on-call
procedures. QUALIFICATIONS: Licensed Vocational Nurse required,
hospice continuous care experience preferred. One year of
supervisory experience Requires knowledge of hospice care, the
dying and bereavement process and demonstrated clinical expertise
in managing patient and family concerns. Certification in hospice
preferred. Proficient in electronic documentation and basic
computer skills (Word processing, Data Entry, Outlook, Word)
Current California driver’s license with proof of insurance
